Home Health for Elderly Patients Reduces Hospitalizations:
Some elderly patients discharged from hospitals either do not have a regular primary care physician or are temporarily unable to visit them due to health or transportation issues. The lack of follow-up and compliance to post-hospitalization care often leads to re- hospitalization and/or a downward health spiral.

Physician Offices & Clinics:
An office or clinic based physician knows that some patients have a difficult time visiting their doctors regularly. In some cases, the patient may have recently fallen, had an exacerbation, or had a surgery which makes transport very difficult. That's where physician offices and Mobile Doctors® can work together.

We work with physician offices and clinics to visit these patients in their home. We will send all visit documentation and prescription information to their offices. If the patient’s health improves to where they can make it to the office, we will transfer the patient back to their regular physician.
